Link utilization comes from statistics polling, not topology polling.  The topology portion only updates NPM's understanding of topology, not interface utilization.  Therefore, if you run topology once then poll for statistics on the interfaces on your maps every 2 minutes, that should take care of it until the topology changes.  The default for topology polling is once every half hour, but you could do topology polling once a week or once a day.

It's a mixture of a number of things, including all that you specified and more.

This is exactly the situation NTM is designed to fix.  You don't know your network topology but (hopefully) have IPs and credentials.  You don't have time to go map all the sites manually, even if it would be helpful to have them.  NTM's goal is to be able to take your IPs and credentials and do all the tedious discovery for you, then let you quickly fiddle around with map parameters to get all the maps you want in a short amount of time.  Is that not working for you?
