As mharvey suggests, you can either copy the out-of-the-box template "Errors in Application Event Log" and modify/rename it to suite your purpose [Settings -> SAM Settings -> Manage Templates]
Or you can create your own application template and add with Windows Event Log Monitor to it [Settings -> SAM Settings -> Create a New Template]
